How to replace your lost license in Arizona
Losing your license or having it stolen can be very stressful and time consuming but it's not. It's actually pretty easy all you have to do is fill out a form and pay the $4 fee.
Follow these easy steps:
- Simply Log on to the online system
- follow the information prompts.
- Your answers will then be compared to your license account information to verify your identity, and then you will receive the duplicate card in the mail. (Please note that you must use a credit card to pay for the replacement license.
Replace your lost license over the phone:
Call (877) 301-8093 during normal business hours (Monday-Friday from 8 a.m.-5 p.m. and Saturday from 8 a.m.-2 p.m.) to speak with a Motor Vehicle Division (MVD) representative. He or she will walk you through the process over the phone.
There are some options below on how to replace your lost license:
Replace your license in Person at a local MVD office to fill out the form and pay the fee in person. You will need to bring valid identification. You must bring at least two pieces, one of which must be a primary source and include your date of birth. If one of the pieces has a photo, you will need only two. If you do not have identification with a photograph, you will have to bring three pieces. All of the pieces must be original or certified copies from the issuing agency and written in English.
